IMPORTANT: Please read all correspondence before completing Licence Application.

Applications can not be processed without payment so please ensure you have included a cheque/money order (payable to BDKC) when returning forms.

Please read AIDKA Licence Application Form thoroughly and ensure BOTH sides of the form are filled out

Correctly / legibly and signed. Please Note: If you are required to have the medical clearance section of the form filled out, it MUST be signed and stamped, or it will be mailed back to you.

You will also need to include with your AIDKA Licence Application

· One jpeg or similar photo Passport size Photo emailed to brent@vinenets.com.au

(if you haven’t already had a photo taken previously)

· Copy of current ambulance cover

· Copy of previous logbook – pages 7, 10 to 12

· New Junior members must provide proof of age (copy of birth certificate)

· For an official licence, all that is required is photo and application form.

 

Without these your licence cannot be processed

 

AIDKA Fees are as follows

· Senior Licence $150.00

· Junior Licence $ 75.00 (only if competing in J Junior or Rookies Class)

· Official Licence $ 50.00

 

PLEASE NOTE:

· Junior Licences: Drivers only competing in either J Junior class or Rookie class

· Rookies must run with Restrictor Plates issued by AIDKA at an extra cost of $15

· All memberships will be tabled at the next committee meeting for approval or rejection as per BDKC
